Disease management is a system of coordinated health care and communication for patients with conditions that require significant self-care. Some of the chronic illnesses treated with disease management programs are high blood pressure, depression, diabetes, and asthma (Leading by Example, 2005). In addition to increased absenteeism and presenteeism, chronic diseases also contribute to turnover, life insurance costs, and workers compensation costs. These and other costs associated with chronic disease can be two to three times higher than an employer’s healthcare costs (Why Invest, 2007).
